We could also have these labels visually hidden in the DOM but still readable by screen readers.

There might be users that are not visually impaired (don’t rely on a screen reader) but have a hard time clicking on a 14x14 pixel area. The labels are helpful in this case.

For the best accessibility, I’d have used a radio-button set, which contains inputs that are semantically-hidden as long as they are disabled. When the radio button choice changes, make sure the user is notified of the change in the page content (aria-live would probably work well somewhere in there).

This doesn’t change the way you activate / select an option / radio button. If you have to click on a 14x14 pixel area then it’s a problem, isn’t it?
